Registration
Before registering with the practice you must complete some forms.
You
can download our form now, please print it out and fill it in before visiting
the practice.

If you wish to join our Practice please bring your medical card along to reception.
If however you have no card you will be given a form at reception. This will be additional to
the forms which will be given to everyone for completion.
The form will ask you for personal details such as your name, address, date of
birth, telephone number, previous address and doctor. This information helps
the
Central Services Agency to quickly trace your medical records and send them
to your
new surgery.
Once you have completed and signed the form your application will be forwarded to
CSA for approval.
After acceptance the CSA will send you a new medical card with your doctor’s name
on for you to keep.
When you register we will give you our practice leaflet containing details about
the surgery.
If you wish to see a doctor you are welcome to make an appointment at the time you
register.
